Not a meeting link you paste into WhatsApp.
The room belongs to the appointment. Before anyone is let in, the server asks five questions — and a no to any of them looks exactly like the appointment does not exist.
Inside the call: screen share, a rear-camera mode for showing something, device and speaker selection, background blur, noise cancellation, and a floating bubble so the call survives leaving the tab. Live Bengali transcription runs alongside, and documents shared in the call land in the patient's permanent record rather than in a chat that disappears.
The consultation is capped at fifteen minutes. It ends itself, and the meeting token expires with it.

Getting paid
Collect the fee before the consultation, not after it.
Approve a request and the patient gets a payment link by WhatsApp and email. The appointment becomes confirmed only once the payment is verified — twice, by two independent paths, so a dropped webhook cannot lose a booking.

Reaching patients

Booking received, approval, the payment link, a reminder ten minutes before, a call-style ring at the slot time, and the prescription itself — over WhatsApp, email, SMS and push.
Every message is logged per channel with whether it was delivered. When a patient says they were never told, you can check rather than guess.
Reminders and the daily digest run on their own schedule. Nobody has to remember to send them.
On the phone you already have
A two-gigabyte Android is the target, not the edge case.
Shifora picks one of three graphics levels before it draws its first frame, then checks that guess against how the device is actually keeping up and steps down if it is struggling. Blur, gradients and motion come off in that order.
This website does the same thing — you can see the control in the footer. It would be a strange claim to make on a page that could not keep it.

Every gate exists at least twice.
Hiding a button in the app is a hint, not a control. The rules that matter are enforced in the database and again on the server, so a client that lies gets nowhere.

Row-level policies scope every table to the practice that owns the row. One practice cannot read another’s patients.
In the procedures
Privileged operations re-check who is calling them, rather than trusting that the caller was already checked.
On the server
Video, payments and messaging independently verify the session, the practice membership and the plan before acting.
